HOW TO APPLY FOR ASSISTANCE

The mission of the LeRoy Butler foundation is to provide financial assistance to women who are struggling financially with their every day expenses while battling breast cancer. To maintain our mission, we have incorporated the assistance of the healthcare providers to aid us in qualifying our recipients in the fast yet confidential manner that protects the applicants HIPPA rights.

In 2012, the foundation began working with providers to partner together to qualify the recipients for the assistance in a manner that is faster, more efficient and confidential. A list of providers that we have established relationships with are listed under "FEATURED PROVIDERS". The assistance application process is simple and will allow for us to assist more men, women and families in need.

If applying through a "Featured Provider":

Click on the link of your provider and you will be directed to a page where you can apply for assistance.  Each provider may have a different process for which they obtain your information for assistance.  Please refer to your provider regarding their process.

Once your provider obtains your information, they will work with the foundation to process your request for assistance.  A representative from the foundation and/or health care provider will contact you to inform you of the decision for your request.


If your provider is NOT listed:

If you wish to apply for assistance and your provider is NOT listed below please contact your health care provider, social worker or RN/Physician to direct them to our website (lbf36.org) to fill out the “Provider Request” form located under links on the "Assistance" page.  Upon receipt of the form, the foundation will contact the provider to process your application and request for assistance. 

Once a decision has been made either a representative from the foundation and/or your health care provider will contact you to inform you of the decision.

What is the criteria for applying?
A person applying for assistance from LBF must have a breast cancer diagnosis, be actively seeking treatment and meet  financial criteria/guidelines.

What is the maximum benefit given to a recipient?

In accordance with the Bi-Laws of The LeRoy Butler Foundation, the foundation can grant a maximum award of up to $800.00 per recipient.  This amount is based on several different factors, criteria and funds available at the time of the request.

How are funds raised by the foundation distributed?
The foundation gives 95% of all funds raised to women and families in need.  The foundation does not hire paid staff to operate the foundation.  The foundation is operated by the generous donation of our volunteer’s time.

If Approved for Assistance, How long will it take to receive a check?
How long it will take an applicant to receive a check depends on availability of funds and the number of women/families that may be on the list prior to an applicant’s approval.  The foundation relies solely on donations, fund raising efforts and sponsorships to be able to provide assistance to our recipients.  Therefore; based on the funds available in the fund this will determine how long it will take for a check to be sent to an approved applicant.  The approved applicants are provided with assistance in accordance to the wait list.

Are there any stipulations as to how an applicant can use the funds?
The foundation does not set any stipulations as to how the funds are used.  The funds are given to the recipient to aid with their every day bills. (For example:  utility bills, rent/mortgage payments, automobile payments, etc.).  Depending on your provider’s relationship with the foundation, the check may be given to the recipient or paid directly to the bill source.  Each situation is viewed individually and is based on a number of factors.
